An incision just within the hairline just above the ear is made. This serves to hide the surgical wound. This continues down along the front edge of the ear, around the earlobe, and then up and behind the ear extending back into the hairline. A similar incision is then repeated on the other side. The skin from the incision line down to the neck and below the chin is then separated from the underlying tissues. Fat and excess tissue are removed while the muscles are tightened. Excess skin from the original incision is trimmed. Finally, the skin is pulled back into place and the incision closed with sutures or staples.

You will also need to take antibiotics for several days in order to prevent infection. Physical activities, especially of the face and neck, will have to be greatly limited immediately after the surgery. To minimize swelling, cold compresses may be used several days after the procedure. Sutures may be removed seven to ten days after the procedure.
If you are taking aspirin or related medications that may interfere with blood clotting, you will need to stop taking this also to prevent excessive bleeding.
